Each organization has the option to change demographic response options on the survey to more closely align with their unique structure, survey more employees or request additional languages. |

We need this list in order to create and send employee surveys. Please provide company-issued addresses. |

Part 1 of the survey process where we collect details about your organization's demographics, benefits and policies. |

We will send a pre-survey IT test to you and your IT contact. This is your opportunity to ensure that the surveys reach your inbox and to review the survey (if you customized, please review your changes). The email will be an actual sample employee survey invitation. |

Complete before survey launches: |
We strongly recommend informing your employees of the upcoming survey. Use the sample provided to let them know the survey is legitimate and that your organization values their timely and honest feedback. |

Employees without an email address can utilize a passcode for survey completion. These are sent to the primary contact for distribution. |

Part 2 of the survey process where we collect employee opinions about the workplace. |

To help increase participation, regularly scheduled reminders will be sent to recipients who have not started the survey. |
